To ensure you are addressed by the name you feel most comfortable with, you can fill out the Chosen Name Request Form. This form allows you to provide your chosen name, which will be used to display on Canvas, faculty course rosters, Outlook email directory, advising in Self Service, and your Middlesex College student ID card. <\/p>\n\n\n\n<p>To make this change, complete the <a href=\"https:\/\/middlesexcollege.edu\/registrar\/wp-content\/uploads\/sites\/19\/2023\/09\/chosen-name-record-update.pdf\">Chosen Name Record Update Form<\/a>.<\/p>\n\n\n\r\n<div class=\"accordion accordion--secondary accordion--secondary-size1 js-accordion\">\r\n  <div class=\"accordion__section\">\r\n    <a href=\"#\" class=\"accordion__head\">\r\n      <h3 class=\"h4 mb-0\">LGBTQ+ Support and Resources<\/h3>\r\n      <span class=\"accordion__arrow\">\r\n        <svg xmlns=\"http:\/\/www.w3.org\/2000\/svg\" viewBox=\"0 0 448 512\"><path d=\"M224 416c-8.188 0-16.38-3.125-22.62-9.375l-192-192c-12.5-12.5-12.5-32.75 0-45.25s32.75-12.5 45.25 0L224 338.8l169.4-169.4c12.5-12.5 32.75-12.5 45.25 0s12.5 32.75 0 45.25l-192 192C240.4 412.9 232.2 416 224 416z\"\/><\/svg>\r\n      <\/span>\r\n    <\/a>\r\n\r\n    <div class=\"accordion__body\">\r\n      <p><ul>\n<li><a href=\"https:\/\/www.rwjbh.org\/rwj-university-hospital-new-brunswick\/treatment-care\/lgbtqia-program\/proud-gender-center-transgender-community-servic\/\"><strong>PROUD Gender Center of New Jersey<\/strong><\/a>: Transgender Community Services (Robert Wood Johnson University Hospital)<\/li>\n<li><a href=\"https:\/\/www.pridecenter.org\/\"><strong>The Pride Center of New Jersey<\/strong><\/a> &#8211; Provides a safe and welcoming space for all individuals who identify as LGBTQ+ to find support, educational tools, health and education resources, and social opportunities within a community of acceptance.<\/li>\n<li><a href=\"https:\/\/www.nj.gov\/dcf\/adolescent\/lgbtqi\/\"><strong>State of New Jersey Department of Children and Families &#8211; LGBTQIA+ Resources<\/strong><\/a><\/li>\n<li><a href=\"https:\/\/saigecounseling.org\/\"><strong>The Society of Sexual, Affectional, Intersex, and Gender Expansive Identities<\/strong><\/a> offers a list of resources for LGBTQ+ individuals and works to educate counseling professionals on LGBT issues.<\/li>\n<li><a href=\"https:\/\/www.aglp.org\/\"><strong>The Association of Gay and Lesbian Psychiatrists<\/strong><\/a> offers numerous resources for LGBTQ+ people who are experiencing mental health conditions, including a directory of LGBT-friendly therapists.<\/li>\n<li><a href=\"https:\/\/lgbthotline.org\/\"><strong>The LGBT National Help Center<\/strong><\/a> provides multiple resources and access to a hotline and a youth chat line.<\/li>\n<li><a href=\"https:\/\/www.glsen.org\/\"><strong>GLSEN<\/strong><\/a> (Gay, Lesbian, and Straight Education Network) provides the <em>National School Climate Survey<\/em> annual report on the experiences of lesbian, gay, bisexual, and transgender youth in U.S. schools.<\/li>\n<li><a href=\"https:\/\/www.pride-institute.com\/programs\/residential-treatment\/\"><strong>The Pride Institute<\/strong><\/a> is an unlocked, LGBT-exclusive facility that offers a residential treatment program, including psychiatric care for depression, anxiety, and other needs.<\/li>\n<li><a href=\"https:\/\/www.thetrevorproject.org\/\"><strong>The Trevor Project<\/strong><\/a> is a multimedia support network for LGBTQ+ youth providing crisis intervention and suicide prevention.<\/li>\n<li><a href=\"https:\/\/therapy.com\/new-jersey\/\"><strong>Therapy.com<\/strong><\/a> connects individuals with licensed therapists nationwide through an easy-to-use online platform. The on-campus <a href=\"\/student-life\/community-resource-hub-food-pantry\/\">Community Resource Hub &amp; Food Pantry<\/a> provides food, personal hygiene items, and cleaning products at no cost to students. The hub also addresses a broader range of basic needs insecurity, such as those related to housing, healthcare, transportation, childcare, technology, WiFi access, etc.<\/p>\n\n\n\n<p>Students may contact Connie Silletti-Cafaro, case manager, with questions about housing and food insecurity: csilletti-cafaro@middlesexcc.edu.<\/p>\n\n\n\n<p>For questions specific to housing in Middlesex County, contact Kayla McCellon, Access Navigator, at 732.668.0989 or kayla.mccellon@co.middlesex.nj.us.
