{"id":445,"date":"2023-11-30T12:23:24","date_gmt":"2023-11-30T17:23:24","guid":{"rendered":"https:\/\/middlesexcollege.edu\/student-life\/?page_id=445"},"modified":"2025-02-05T10:03:33","modified_gmt":"2025-02-05T15:03:33","slug":"documentation-guidelines","status":"publish","type":"page","link":"https:\/\/middlesexcollege.edu\/student-life\/disability-services\/documentation-guidelines\/","title":{"rendered":"Documentation Guidelines"},"content":{"rendered":"\n<p>Documentation is required by Middlesex College to establish and support the need for reasonable accommodations and academic accommodations.<\/p>\n\n\n\n<p>All documentation is reviewed on a case by case basis. The Center for Accessibility and Disability Resources utilizes documentation in conjunction with a personal intake interview to clarify and understand the disability and true impact on an individual\u2019s academic life.<\/p>\n\n\n\n<p>The College may deny an accommodation, academic adjustment or request for assistive technology, if the modification fundamentally alters an academic program. The purpose of requesting documentation is to establish protection from discrimination. Please be advised that at all times the College reserves the right to request additional documentation.<\/p>\n\n\n\n<p>All documentation is considered confidential material and is secured in locked file cabinets with limited access only by professionals who have a need to know. Students should make a copy of their own records prior to submitting them. The College keeps documentation for five years after a student graduates or leaves the institution.<\/p>\n\n\n\n<p>&nbsp;Also, the College does not do any diagnostic work for students and all students requesting accommodations and services are responsible for providing appropriate and timely documentation.<\/p>\n\n\n\n<p><strong>Documentation should include the following:<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>All documentation should be current and tested according to adult norms. A diagnostic statement should identify the specific disability.<\/li>\n\n\n\n<li>A description of diagnostic tests and or criteria used should be provided. Test results and a narrative should also be included.<\/li>\n\n\n\n<li>Credentials of diagnosing physician, psychologist, learning specialist, audiologist or professional should be clearly stated.<\/li>\n\n\n\n<li>The documentation needs to show the functional impact of the disability on learning or other major life activity and the degree to which it impacts the individual.<\/li>\n\n\n\n<li>Statement indicating medications, treatments, or any Assistive Technology devices currently prescribed or used should be included.<\/li>\n\n\n\n<li>Descriptions of any accommodations used at the secondary or post-secondary level.<\/li>\n\n\n\n<li>IEP\u2019s or 504 plans may be useful but are not, in and of themselves sufficient documentation to establish the need for accommodations.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading bb-primary-1\">Guidelines for Specific Disabilities<\/h2>\n\n\n\n<p>Below please find additional information for several frequently identified disability groups. This list is not meant to be exhaustive, however, and there are other disabilities that may not have been included here.<\/p>\n\n\n\r\n<div class=\"accordion accordion--secondary accordion--secondary-size1 js-accordion\">\r\n  <div class=\"accordion__section\">\r\n    <a href=\"#\" class=\"accordion__head\">\r\n      <h3 class=\"h4 mb-0\">Learning Disability<\/h3>\r\n      <span class=\"accordion__arrow\">\r\n        <svg xmlns=\"http:\/\/www.w3.org\/2000\/svg\" viewBox=\"0 0 448 512\"><path d=\"M224 416c-8.188 0-16.38-3.125-22.62-9.375l-192-192c-12.5-12.5-12.5-32.75 0-45.25s32.75-12.5 45.25 0L224 338.8l169.4-169.4c12.5-12.5 32.75-12.5 45.25 0s12.5 32.75 0 45.25l-192 192C240.4 412.9 232.2 416 224 416z\"\/><\/svg>\r\n      <\/span>\r\n    <\/a>\r\n\r\n    <div class=\"accordion__body\">\r\n      <p><p>Students with a learning disability are required to provide a comprehensive psychological and educational evaluation.<\/p>\n<p>Testing should be comprehensive and include current measures of aptitude and cognitive ability (e.g. WAIS-R) and achievement (e.g. Woodcock Johnson ) with current levels of functioning in reading, math and written language) and information processing deficits. A qualified professional must conduct the evaluation and include all standard scores, sub-test scores and percentiles using age based norms.<\/p>\n<p>Please note that grade equivalents are not acceptable. A diagnosis must be present and not individual learning styles, learning differences, academic problems or test anxiety, which by themselves would not be considered cognitive disabilities for which accommodations are appropriate. The assessment report should include recommended academic accommodations.<\/p>\n<p>A neuropsychological assessment is also acceptable.<\/p>\n<\/p>\r\n    <\/div><!-- \/.accordion__body -->\r\n  <\/div><!-- \/.accordion__section -->\r\n<\/div><!-- \/.accordion js-accordion -->\n\n\r\n<div class=\"accordion accordion--secondary accordion--secondary-size1 js-accordion\">\r\n  <div class=\"accordion__section\">\r\n    <a href=\"#\" class=\"accordion__head\">\r\n      <h3 class=\"h4 mb-0\">Attention Deficit Disorder (AD\/HD)<\/h3>\r\n      <span class=\"accordion__arrow\">\r\n        <svg xmlns=\"http:\/\/www.w3.org\/2000\/svg\" viewBox=\"0 0 448 512\"><path d=\"M224 416c-8.188 0-16.38-3.125-22.62-9.375l-192-192c-12.5-12.5-12.5-32.75 0-45.25s32.75-12.5 45.25 0L224 338.8l169.4-169.4c12.5-12.5 32.75-12.5 45.25 0s12.5 32.75 0 45.25l-192 192C240.4 412.9 232.2 416 224 416z\"\/><\/svg>\r\n      <\/span>\r\n    <\/a>\r\n\r\n    <div class=\"accordion__body\">\r\n      <p><p>Students with Attention Deficit Disorder are required to submit documentation that substantiates a diagnosis of AD\/HD.<\/p>\n<p>Documentation should include evidence of impairment, relevant testing information and a summary which included recommended academic accommodations by a qualified professional.<\/p>\n<\/p>\r\n    <\/div><!-- \/.accordion__body -->\r\n  <\/div><!-- \/.accordion__section -->\r\n<\/div><!-- \/.accordion js-accordion -->\n\n\r\n<div class=\"accordion accordion--secondary accordion--secondary-size1 js-accordion\">\r\n  <div class=\"accordion__section\">\r\n    <a href=\"#\" class=\"accordion__head\">\r\n      <h3 class=\"h4 mb-0\">Psychological Disability<\/h3>\r\n      <span class=\"accordion__arrow\">\r\n        <svg xmlns=\"http:\/\/www.w3.org\/2000\/svg\" viewBox=\"0 0 448 512\"><path d=\"M224 416c-8.188 0-16.38-3.125-22.62-9.375l-192-192c-12.5-12.5-12.5-32.75 0-45.25s32.75-12.5 45.25 0L224 338.8l169.4-169.4c12.5-12.5 32.75-12.5 45.25 0s12.5 32.75 0 45.25l-192 192C240.4 412.9 232.2 416 224 416z\"\/><\/svg>\r\n      <\/span>\r\n    <\/a>\r\n\r\n    <div class=\"accordion__body\">\r\n      <p><p>Students with a psychological disability are required to submit documentation by licensed mental health professional that includes a clear statement of the disability including the DSM-IV diagnosis, summary of present symptoms and how they impact on a student\u2019s ability to learn in an educational setting. The material should also include recommendations for academic accommodations.<\/p>\n<p>Medical information relating to medications prescribed and their impact on student\u2019s ability to learn should also be included.<\/p>\n<p>Given the nature of psychological disorders, students may be asked to provide updated information every six months.<\/p>\n<\/p>\r\n    <\/div><!-- \/.accordion__body -->\r\n  <\/div><!-- \/.accordion__section -->\r\n<\/div><!-- \/.accordion js-accordion -->\n\n\r\n<div class=\"accordion accordion--secondary accordion--secondary-size1 js-accordion\">\r\n  <div class=\"accordion__section\">\r\n    <a href=\"#\" class=\"accordion__head\">\r\n      <h3 class=\"h4 mb-0\">Physical\/Medical Disabilities<\/h3>\r\n      <span class=\"accordion__arrow\">\r\n        <svg xmlns=\"http:\/\/www.w3.org\/2000\/svg\" viewBox=\"0 0 448 512\"><path d=\"M224 416c-8.188 0-16.38-3.125-22.62-9.375l-192-192c-12.5-12.5-12.5-32.75 0-45.25s32.75-12.5 45.25 0L224 338.8l169.4-169.4c12.5-12.5 32.75-12.5 45.25 0s12.5 32.75 0 45.25l-192 192C240.4 412.9 232.2 416 224 416z\"\/><\/svg>\r\n      <\/span>\r\n    <\/a>\r\n\r\n    <div class=\"accordion__body\">\r\n      <p><p>Students with physical and medical disabilities are required to submit documentation from physicians or medical specialists with an expertise in the area for which accommodations are being requested.<\/p>\n<p>Documentation should include a clear statement of the diagnosis, the impact the disability has on a student\u2019s ability to function in an academic setting and recommended accommodations.<\/p>\n<\/p>\r\n    <\/div><!-- \/.accordion__body -->\r\n  <\/div><!-- \/.accordion__section -->\r\n<\/div><!-- \/.accordion js-accordion -->","protected":false},"excerpt":{"rendered":"<p>Documentation is required by Middlesex College to establish and support the need for reasonable accommodations and academic accommodations.
