Failure to comply with this requirement will prevent you from registering for your next semester.<\/p>\n\n\n\n<p>Call the health office at 732.906.2530 for more information.<\/p>\n\n\n\n<h2 class=\"wp-block-heading bb-primary-1\">Required Immunizations<\/h2>\n\n\n\n<ol class=\"wp-block-list\">\n<li>Measles (2): The first dose must have been given after 1968 and after your first birthday. The second dose must have been given at least 30 days after the first dose.<\/li>\n\n\n\n<li>Mumps (1): Given after your first birthday.<\/li>\n\n\n\n<li>Rubella (1): Given after your first birthday.\n<ul class=\"wp-block-list\">\n<li>The vaccine may have been given for each disease separately, or it may have been given in combination form (MMR).<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Hepatitis B (3): A series of three shots given over a six-month period or Heplisav.<\/li>\n\n\n\n<li>Meningitis \u2013 ACWY. Students \u2013 full and part time. One dose administered between 11 and 12 years old; booster dose at 16 years. Persons 16 to 18 years with no prior vaccination are recommended to receive 1 dose. Your name and birthdate must be on the record and include your student number on the form.<\/p>\n\n\n\n<div class=\"wp-block-buttons is-layout-flex wp-block-buttons-is-layout-flex\">\n<div class=\"wp-block-button btn btn--size-3\"><a class=\"wp-block-button__link wp-element-button\" href=\"\/student-life\/wp-content\/uploads\/sites\/9\/2023\/09\/evidence-of-vaccinations.pdf\">Evidence of Vaccinations Form<\/a><\/div>\n<\/div>\n\n\n\n<h2 class=\"wp-block-heading bb-primary-1\">Exemptions<\/h2>\n\n\n\n<p><strong>Two exemptions<\/strong>&nbsp;are allowed and must be certified as prescribed by law. They are:<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li>Medical reasons, such as immunosuppression issues, pregnancy or allergic reaction to neomycin; or<\/li>\n\n\n\n<li>Religious beliefs.<\/li>\n<\/ol>\n\n\n\n<p>You must fill out a request for exemption for either reason.&nbsp;If a medical exemption, a note from the physician is required stating the nature of the medical condition that is preventing the immunization and the time after which the student may receive the vaccine, unless it is a permanent condition, such as being allergic to the vaccine.<\/p>\n\n\n\n<p>For a religious exemption, you must fill out a request for exemption and state how the administration of the vaccine interferes with your religious beliefs.<\/p>\n\n\n\n<div
